In order to qualify for the Apple iPad give-away, you must be one of the first five orthodontists who: (1) fully completes and transmits the practice assessment request form to OSI via the link provided above; and, (2) participates in a phone interview with an OSI Business Consultant who will obtain financial and operational information regarding the practice (OSI will sign a Confidentiality and Non-Disclosure Agreement with you to protect such information), including two years audited income statements, previous 90 days of purchasing invoices, and practice new case start data and conversion rates for previous 12 months; and (3) participates in a presentation by OSI which reports its findings of the Practice Assessment containing and an action plan.
